The Canadian Red Cross Society, a non-profit, humanitarian organization dedicated to helping Canadians, as well as the most vulnerable throughout the world, is currently seeking a Coordinator for the Priority Assistance to Transition Home (PATH) program. The PATH Program is designed to facilitate a smooth transition from hospital to home and ensures that community support services are in place for clients.
Reporting to the Senior Manager of Operations, the successful candidate will be responsible for the day to day coordination of the PATH program in the District.
Responsibilities include :
Coordinate, implement, and evaluate the delivery of the PATH Program.
Conduct client assessments;
Ensure excellent client service and maintain positive relationship with other agencies and Red Cross departments;
Develop implementation plans for program delivery;
Respond to client inquiries;
Follow up on client complaints adhering to the National complaint process;
Complete program administration and provide statistical reports;
Evaluate service delivery and make recommendations for improvement;
Recruit, train, and coordinate Transitional Care Workers
Maintain alignment with assigned budget;
Perform accounts payable duties as appropriate; and
Adhere to National Standards in the delivery of the program.
Liaise with other community agencies/organizations regarding related services/initiatives to foster mutual understanding, coordination, and partnerships.
Research the market for growth potential of service/initiative.
Integrate volunteer resources within the job’s scope of responsibilities.
Adhere to implemented strategies to support Social Justice & Diversity in accordance with the Society’s policies and principles.
Follow all policies and procedures as required to maintain a healthy and safe working environment.

Knowledge and Skills:
Bilingual: French and English required
Supervisory or project management experience, including an ability to lead, motivate, and evaluate;
Ability to develop, implement, and evaluate program plans;
Excellent skills in risk, crisis, and conflict management;
Strong customer service skills;
Exceptional telephone skills and interpersonal skills;
Scheduling experience as asset;
Ability to work independently, prioritize, multi-task, and achieve successful results;
Knowledge of medical terminology and documentation is an asset;
Current CPR/First Aid certification;
Computer proficiency in word processing, spread sheet, and data base applications (Word, Excel, Power Point, and CIMS);
A valid driver’s licence and access to a reliable vehicle.
Education and Experience:
The minimum qualifications for this position are one to three year’s community college diploma in Gerontology or related and between one to three years job related experience or an equivalent combination of education and experience. Good communication/presentation skills, leadership abilities, and experience working with clients and volunteers is essential.
